Wiki ED admits to OBS, IM admits to Inpt same day

Our ED providers admit a patient to observation, then an IM or Hospitalist come in the same day and admits to inpatient. Medicare is denying our admit to obs because of the admit to inpt by a different providers. Any suggestions?

ED Visit

I'm assuming the Observation Service was was coded but not the ED visit since the patient was admitted to Observation on the same day. You still have the option of coding the ED visit prior to Observation.
