If one physician takes over for another physician in the ED, does the physician taking over have to write something like an attestation to state that he/she agrees with the first physician's note? Especially if the 2nd physician comes in and just states the impression?

Yes

They should write and initial and date a note. It should reflect exactly what they did. What was the basis od the impression? Did they reexamine the patient, read test results etc? In this case it looks like you would be billing under the initial physician who apparently did most of the work. Split/shared doesn't apply to two physicians.
