Patient has HTN and CKD. Physical exam (Outpatient primary clinic) edema was noted and documented in Assessment and Plan. Can I code edema seperately or is it included with HTN/CKD??

Same scenerio with CHF also.

When do/don't code edema seperately?

I would like to get some feed back
