Does anyone have an official source as to what the coding rules are for an EGD with a stent removal and a stent placement? ERCP has a code that figures in the work for both by building into an 'Exchange' code. EGD does not have an 'exchange' code. I can find no official sources saying we cannot bill both together. I would assume you can bill both due to the risk and work associated with the procedures; But someone please let me know if you have an official source on this: CMS, AAPC, CPT guidelines, etc. I may have just not been able to find it.
 
If all that was done, was the removal. You'd code CPT 43247, when it's a replacement. Then you'll just code CPT 43266............
