Question regarding E&M and Mod 25 in Primary Care. We often complete an EKG in our RHC Primary care office when filling out preop clearance forms for our patients. We are not the surgeon completing the surgery or the provider who determined the surgery was necessary, just their PCP. My question is how do my providers code for these? The E&M code for the visits is a standard 99213-99215
