What would be the differnce is using V23.81 (Screening for high risk pregnancy, elderly primagravida) and 659.53 (Other indication for care or interventuion related to labor and delivery, elderly primagravida?

It looks to me like we would use the screening code for the initial diagnosis for th pregnancy and would only change it to 659.53 once there is actually an indication of a problem. Could someone verify this for me?
