Wondering what your thoughts are on RX mgmt for ER e/m leveling.

If the patients med list is included in the dictation and nothing is written about provider reviewing or change any of their meds, does this still qualify as rx mgmt by simply just having the medication list on the dictation?

What are your thoughts?

I am being told that when there is a list of medications listed on an ER encounter, i am to consider that "moderate risk" because that is considered prescription drug management. So, therefore that would bring the E/M level to a 3 or 4 depending on the rest of the documentation. I am thinking we need to have more documentation regarding the prescription drugs, i.e. to continue them, change in dosage,etc.
