Can anyone tell me what the required documentation for cpt code 95832 is? I believe what the provider is documenting is sufficient but want to be sure.
The provider is documenting as to why the patient is having the test done, then the report lists each area tested and it's findings. The provider then documents what the finding were.

Is this being documented correctly?
