I have a few denials from BCBS that I’m hoping someone can offer some guidance on. We billed 95909 and 95886 with a dx of G56.01 (CTS). The denial/appeal states this has to be a limited emg study (95885) due to the dx. I can’t seem to locate a policy, LCD or any guidance that states this.
