Help! I am beginning to receive denials from Medicare and Healthspring Plans for: Missing or Invalid Modifier on the 95886. I have reviewed the cms guidelines and I am not locating which Modifier is needed. The only thing I can find related to a modifier is if an office visit is also done at the time of service to add -25. If you can share the guideline for the modifiers required I would appreciate it.
