Wiki Endovenous ablation 36475 & 36476

Can 36476 only be used when performed on the same extremity (RT/LT) as 36475? The issue I'm dealing with is 1 ablation was done on Lt leg and another on Rt leg. Do modifiers need to be used and/or the add-on code? Thanks!!

It would be 36475,50 (or 36475,LT & 36475,RT, depending on payor policy)
36476 is used if it's for another vein on the same leg. Also, only 1 unit of 36476 can be reported per leg regardless of how many additional veins are treated.
