Per the CPT manual, you cannot report CPT 36478 with CPT 36475 & CCI supports this & indicates that modifier -59 is NOT allowed. How do you then code endovenous ablation of two separate veins (different entry sites) when one vein is treated with LASER ablation & the other is treated with RADIOFREQUENCY ablation? Can anyone help with this? :confused:

You can't if they are in the same surgical field.

I've never had a report where both techniques were used; my physician decide on one or the other, and from there, decide to treat additional veins and we utilize 36476 or 36479, respectively.

I would bill the cpt representing the highest RVUs since both were performed in the same surgical field.
