Hello,
I have a question on coding an Ependymoma brain mass, grade two, status post resection. Upon researching this I found that grade 1 and 2 are non cancerous and grade three is cancerous. Does the doctor need to specifically document that it is benign in order to code as such? When I look under the table of neoplasms, it gives the code D33.0 for benign and D43.0 for uncertain behavior. When I look up a history code, all I see is Z85.841 which is personal history of malignant neoplasm of brain but documentation doesn't state that it is malignant. I am not sure ho to code it.
