A patient had an epidural blood patch under anesthesia. The MDA put the code for the patch (62273) in the anesthesia record. Is it okay to bill/code just from the record or does the provider still need a separate procedure note detailing the process?
 
Probably need a brief description of the procedure written by the provider where there is some blank space on the anesthesia record if he is not going to do a formal procedure note.
