1. Are transforaminal (64483) and interlaminar (63211) epidural injections billable on the same date of service?

2. Denials for lumbar epidurals example: L4/L5/SI (64483, 64484) and sacroiliac joint injections (27096) - same date of service - (separate anatomical sites and injections). Payers are denying claims when performed on the same date of service "not usual or customary, normally procedures are not performed together"). Anyone else getting these codes paid together?

3. Osteoarthritis Knee:: hyalgan (gel syn) type injection - new code J7328, old code J7321 - LCD approved for M17.0 - fee schedule paid at $0 from Medicare - How to get reimbursed for meds by payer? (we are assigning ABNs to PT responsibility). Any suggestions?
