I need help with coding this operative note:

Patient presented to the lab in sinus rhythm. The right femoral vein was then cannulated with an 8 French and 7 French sheath. A decapolar catheter was passed and positioned in the coronary sinus. The 7 French sheath was replaced within a deflectable sheath, The righ atrium was then mapped during pacing from the proximal coronary sinus. Conduction block across the tricuspid isthmus was noted. While mapping he spontaneously went into atrial fibrillation. He cardioverted with restoration of sinus rhythm. However, with 1 minutes time went back into atrial fibrillation. A bolus of amiodarone 150 mg was administered IV and was again cardioverted. He maintained sinus rhythm for a couple of minutes before going back into atrial fibrillation. The procedure was then concluded the sheaths and catheters were removed from the patient digital pressure applied for hemostasis.
