Can we bill for an er visit and an inpatient visit? Same provider and same date of service.

not if it is the same patient, you may combine the ER encounter with the inpatient, to arrive at one visit level.

Sort of a long the same lines, patient was seen in the ER by our podiatrist due to a foot injury after being stepped on by a horse. He then performed surgery that day. Can our doc bill an ER visit along with surgery on the same day?

it is possible depending on the documentation. A slong as you meet the criteria of significant and separately identifiable evaluation then you can bill the E&M with a 25 modifier.
