Wiki Er with admission-modifier?

Our doc was called to the er and did a full evaluation & work up-then decided to admit the patient. Is there a modifier to get paid for both? I know you can technically only bill one e/m visit per day but this was two seperate episodes of care for the same problem.
Any advice would be greatly appreciated!

Initial Hospital Visit

All E/M services provided on the date of hospital admission will be rolled into one Initial Hospital Visit level of service.
