Wiki ESI and Fluoroscopic Guidence.

In the CPT book 77003 Fluoroscopic guidence states that it is included in codes 62310 and 62311. Last year we had BCBS come back and say that we HAD to bill the 77003. We have been billign it that way since and have been paid. We also had a webinar that stated we could still bill the fluoroscopic code 77003 for 2012. We are very unsure which is the correct way to bill. Does anyone have any ideas?

i was confused at first myself, but we figured out that YES you can bill the 77003 with the ESI injection, the one that changed for me is that i can no longer bill the 77003 with the SI joint injection...and of course from last year the facet joints.

The CPT book does not say that 77003 is included in 62310 and 62311. As a matter of fact it specifically states that "Fluoroscopy (for localization) may be used in the placement of injections reported with 62310-62319, but is not required. If used, fluoroscopy should be reported with 77003."

What many people see is this statement "Injection of contrast during fluoroscopic guidance and localization is an inclusive component of 62263, 62264, 62267, 62270-62273, 62280, 62282, 62310-62319." That statement doesn't mean you can't code for fluoro guidance, what it means is that you can't code separately for the injection of contrast - in other words you can't code 96372 for the injection of contrast, 77003 for fluoro, then 62310 for the pain injection. The next statement after that is "Fluoroscopic guidance and localization is reported with 77003..."
