Here is the scenario: Pt comes in for yearly transplant evaluation for possible kidney transplant. Pt's natural kidneys have been removed due to polycystic kidney disease. Pt had a kidney transplant but that kidney was also removed due to rejection. Yes the patient has no kidneys and is on dialysis. Dr. dictates ESRD secondary to polycystic kidney disease. How do you code it? Please supply any documentation as to where you got the information on how to code it.
 
I found this 2013 response from the third quarter 2013 Coding Clinic - it's from the time of ICD-9, of course, but I imagine the guidance would still be relevant:

When a patient with a history of polycystic kidney disease (PKD) presents status post kidney transplant, should the PKD still be reported? At our facility, transplant patients typically receive a unilateral renal transplant and the original kidneys are not removed from the body. Should the patient's history of polycystic kidney disease be reported as an additional diagnosis for these post transplant patients?

Answer:

Assign code 753.12, Polycystic kidney, unspecified type, with code V42.0, Organ or tissue replaced by transplant, Kidney, to describe the transplant status. Although the patient had a transplant, the patient still has the condition. Polycystic kidney disease is a systemic condition that can affect other organs. Therefore, it is appropriate to assign a code for the polycystic kidney disease.
