We code for ESRD dialysis treatments for a facility in Texas. Novitus is the new Medicare Intermediary (took over for Trailblazers) and is questioning and requesting an additional code to 276.69 that the physician has always documented to account for an extra treatment due to interdialytic weight gain. They want to know the "why" (which could be a number of reasons like drinking alcohol, eating something too salty, etc). It could be that Novitus is just incorrect in their understanding of the needed code because we have been unable to find anything else to use.

Does anyone else have any recommendations?

Appreciate any help anyone can provide.
