Is there any guideline stating that N18.6 ESRD or any CKD codes can not be used alone? I have researched and with my understanding this category N18.- may NOT be used as primary but must follow the etiology code. But could be used alone if not primary as long as the patient doesn't have hypertension or Diabetes.
 
No, there is no Official guideline that forbids a code from category N18 from being coded alone or from being the principal or first-listed code.

If there is an etiology, then coding rules say the etiology must be coded first.

If you are getting denials, this may be a payer requirement and not a coding rule.
