Does anyone know if a provider has the choice to either bill as a Monthly Capitated Payment code (90951-90962) OR just billing an office visit + daily ESRD code (90970)?

I can't find any regulations that say you MUST bill one way or the other, but I don't know if it is implied that they will bill with MCP codes only.
