Some confusion on CPT 90960 (four or more face to face visit per month)
> should this be billed in units: if patient was seen 5 visit - billed 5 units or 1 unit
 
Per CPT instructions, the ESRD codes are reported once per month, so one unit only. The reimbursement for one unit pays for all of the visits in the code description and most payers will deny any additional units billed during the same month.
