HELP! Where are my SUPER DUPER GYN Coding EXPERTS!!

I faxed a sterilization consent form to Texas Medicaid for a patient that received an ESSURE for birth control.

The form returned with the following errors:

NAME OF PROCEDURE MUST BE THE SAME IN ALL PLACES.

We wrote in ESSURE (TUBAL LIGATION).

Should it be worded different? Should we just write ESSURE on all lines throughout?
