The physician has seen the Patient in Hospital,and next visit he performed in his office, both are under same Group. I have a doubt whether we can consider this patient is new or established pt.

The office visit is established. The POS of the first encounter isn't relevant. If the provider has seen the patient face to face within the last 3 years in ANY setting, the patient is now established.
