Wiki Established visits=all=99214

Can a provider (INTERNIST)codes all his established visits ALL= 99214 for all follow-up of chronic medical problems (more than 3 Dx"S most of the time) in a monthly basis although sometimes he just continue and changed the Rx.
As a CODER, am I not in jeopardy of an Audit eventhough he always has good documentations or it is not an abuse for these visits to use 99214 all the time just to increase the revenue?

I would be very careful of this. Regular audits of the documentation should be performed on these services.

I also would be very careful with this, that is a sure target for an audit. make sure to do regular in-house audits to make sure all bullets are meet in order to bill the e/m. Remember RAC is comming

Not only do you need to make sure all the "bullets" are there for a 99214, but they must also meet medical necessity for this level.
