Can a provider that was requested to do a NCS only bill an evaluation and management code?

Only if the patient complained about something that is completely separate from the NCS. If everything pertained to the NCS then there is no E/M.

why would the provider not want to bill the NCS?

I think the OP was referring to having the request for only NCS, as opposed to maybe NCS and consult...not only billing for NCS ?
