One of our Docs excised a 12 cm cyst on the back. Do I code this as 11406 X3 with a modifier 22?

11406 is for over 4.0 cm so you would only report it once. You would only append the modifer 22 if the service was unusual or complicated and that would have to be clearly stated in the notes.
I would look at the notes and see fi there was a layered, intermediate or complex closure of the defect created by the excision. That would be seperately reportable with a repair code or a reconstructive closure code.
