E/M Exam elements question

For the 97 guidelines under exam when it says for example:

Description of speech including: rate, volume, articulation, coherence, and spontaneity with notation of abnormalities

Does the provider have to document all of the elements bolded? Or just one? Any help is greatly appreciated.

Any number of + or - descriptors within the "Speech" bullet is an indication of assessment of "Speech" as part of the Psychiatric Exam.
