I have a question and being argued with I was wondering if anyone can help.

If in the exam under Consitutional the provider states: The patient is alert and in no acute distress, appears well nourished.

Would you give credit for this under.. Just Constitutional or Constitutional and Neuro or Constitutional and Psych.
