If we have a patient that has seen an optom then referred the patient to our Optomeitrist who was taken to our Ambulatory center for a yag. Can the doctor bill for an exam with the yag in the ambulatory center?

If the referring optometrist is not in the same practice, and the optometrist who did the YAG saw the patient and made the decision for surgery, then you should be able to bill the Exam with modifier -57. If, however, the referring optometrist made did the pre-op/hx and made the definitive decision for surg and the surgeon did not examine the patient to determine the need for surg, then you cannot bill it.
