the example provided in the text reads: "

When procedure code 94610 is reported with another procedure, modifier 51 is not appended to 17004." Also, "Codes that are exempt from modifier 51 are procedures that are typically performed with other procedures" What does CPT code 94610 have to even remotely do with CPT 17004? I can't imagine a single situation where these two procedures owuld "typically be performed with (each other)".
