List, I should know this, right? Where is a good place to go for a sample of good documentation for multiple injections for pain control? We're starting to see denials come in and upon deeper inspection per visits with insurance reps, know that the documentation is lacking. I've made some initial attempts to tighten the gap, but would love to know where I might find some samples, free or with fees. Other than WHERE the injection is given, and PURPOSE and a clear example of how many, what else is needed? Doctor asked if we could find a good chart note/example as most likely we'll be fighting for payment for the multiple admins. ---Suzanne E. Byrum CPC
