Hello, New to coding day surgery for facility. Have benign skin lesion excised full thickness w/ measuremnt for code 3.06 cm. Now is that 11404 or 11403? The measurement gap in the code description is my issue. I am thinking we did not reach 3.1 cm so it's 11403. Would appreciate your feedback. Thank you,
