Hi - I have an Operative report stating excision 0.5 nevus left upper back, 5 dermal fibromas each .5 cm on back, 0.5 cm nevus left axilla.
the nevus upper back was excised with layered closure (12031/11400); The derma fibromas were excised and cauterized and the nevus axilla was simple removal. Since these are all in the same area, do i bill : 12031,11400,11400x5 XS, 11400 XS? . The path report shows - benign nevus axilla and upper back and intradermal nevi x5. Thanks
