Excision of lesion that extends to a different area.

What would be the proper code for excision of a benign lesion (over 4.0 cm) that is on the patient's shoulder but extends onto his neck? Would it be 11406 (trunk) or 11426 (neck)? I queried the doctor and was told half of it was on the shoulder and half on the neck! :confused:
