If a physician removes a lesion that is 4x5 cm, how would I figure the actual size of the lesion?

Please read the guidelines in CPT - How to measure lesions is described in detail, including diagrams. (2010 Professional edition begins on page 59 and continues through page 61.)

Lesions are coded by diameter plus narrowest margin.
