Excision upper back previously Biopsy 9mm, margin 5mm stating total excised w/margin as 1.9cm. this is also with intermediate repair.
Am I going as excision OR shaving ?

This seems clearly to be an excision. (I assume there is more in the note that gives more details about how it was excised.) Unless I'm missing something, there would be no reason to consider billing it as a shave.
