when coding for the excision nevus on conecting areas in CPT such as the lip, nose, eylid etc but all with different measurments such as lip 1.5 nose 4.0 ears 1
do I add the cm together and code one code or do i code each of them seperate

You should wait for the path to make sure that they are not malignant. If they are benign 1.0 ear would be 11441, 1.5 lip would be 11442, 4.0 nose would be 11444. You will have to put 59 modifiers on the 11441 and 11442. If there are margins documented you could add those and recode appropriately.
