It's been my experience that exploratory laps are considered approaches once anything else is done.

Yes, this was all done on same encounter.
So what you are saying is that exploratory laps are included in open procedures of the abdomen, and that I would code only for the cholecystectomy?

if a significant amount of time was spent on the lysis of adhesions you can put a modifier 22 on the gallbladder and request more money - will probably need to send a note and the op report.
