Expl Lap w/omental patch closure of perforated duodenal ulcer

Upper midline incision and carried through subcutaneous tissues and fascia. Abdom entered, note significant amount of fluid, evacuated.

Began expl in right upper quad, upon eval of liver a perforated duodenal ulcer was identified. The perforation was on the anterior aspect of first portion of duodenum. Healthy omentum was identified and freed. Anchoring sutures placed on either side of duodenum and omental tongue sutured into place. exploration continued. Bowel was run from ligament of treitz to ileocecal junction, no evidence of injury in small bowel. Colon examined from cecum to sigmoid, no other lesions noted. Gallbladder had minimal inflammation. Abdom irrigated, closed, dressed.
