Redo thoracotomy
Dense lysis of adhesions spent 2 hours to dissect down to tumor
Wedge resection of the right upper lobe.
Without typing whole op report this is the gist of it. My surgeon mentions all of these in his report. My question is can I code the lysis of adhesions, it was extensive? Or is it considered part of the wedge resection?

I would try to bill it. Especially with strong documentation that this was over and above usual dissection time. A note in box 19 would be necessary and the payor may ask for the report anyway, but worst case scenario is that it has to be written off as a bundled service.
