Is anyone having difficulty getting their medical staff to document injury cases to include external causes? I'm stuck between trying to enforce proper documentation and wondering whether its even important enough to follow up on. I know its not a REQUIREMENT but I've already had to send office notes to Humana and BCBS on some visits where it wasn't provided so I couldn't code it. Is this a difficulty that is common for everyone?
