S/P Aortic Valve Replacement, doing well post surgery,
SKIN: Forehead lesion she is concerned about
MEDS: PT concerned about about how many she is having to take, discussed them and she is ok with taking them for now
EDEMA; RT leg persistent edema (vein stripping from this area)

A/P: Chronic Kidney disease unspecified (585.9) labs pending, should be improved post valve replacement,
Aortic Valve Disorder (424.1) s/p replacement
Unspecified essential Hypertension (401.9) well controlled
Unspecified acquired hypothyroidism (244.9) on lower dose, will monitor
MY QUESTION: Should I code this as V43.3 (arotic valve replacement, V58.73 (f/u), 404.90, and 428.0 due to hypertension/CKD? this is a medicare patient.
