Looking for clarification..! dr documented moderate neurocognitive impairment with behavioral disturbances. I came up with F02.81..??

Dr mentions throughout the chart new diagnosis of dementia. Pt in 70s, denying diagnosis. Recommend neurology follow up post d/c.
 
I'm pretty sure your choices are mild cognitive impairment, and major cognitive impairment. I could be wrong, but I don't think there's a "moderate".

Some doctors use the shorthand of "dementia" to mean any decline in higher function. Unless a provider is certain it is dementia, then we use R41.9, unspecified symptoms of cognitive function and awareness.
