If the physician states they spent 45 minutes with a patient and also performed an office procedure during this time are you still coding level 4 office visit with a procedure?

Any help would be much appreciated.

I would have to see documentation to give a definite answer, but gut instincts say NO.
 
Face to face time would only be the amount of time spent on the E&M. If you included the time for the procedure also, you would be billing twice for the procedure.......once for the time and once for the procedure itself.
