Facetectomies and Foraminotomies without Laminectomy

Would I bill an additional 63048 for the facetectomies and formaminotomies?

OPERATION: L4-L5 laminectomy with medial facetectomies and
foraminotomies, bilateral L3-4, L4-5, L5-S1 and the S1 nerve
root.

I think it would be 63047x1 and 63048x1.

Any feedback would be appreciated.

Without the full op note, it looks like 63047 would be your code. You can bill 63048 up to 5 times based on the MUE's as long as your op note supports it. So 63047, 63048, 63048-59, 63048-59 (or XS if you are billing Medicare).
