Wiki Facility billing unlisted codes for OP time level 2 or 3

The facility charge has an unlisted code and The description says “OP time level 2 30 minutes” or “OP time level 3”. The provider then bills a professional claim with a cpt code for the procedure performed. Why is the facility billing OP time and why are they using an unlisted procedure code?
