Wiki Facility New Patient or Established

I am trying to set up guidelines for when a patient is new to our facility or Established. We are billing out the facility charge for Radation Therapy, which is part of a hospital. I am wondering would the patient be considered new if they have never been to our department or do they have to be new to the whole facility.
